idea what to do" are a kind of rite of passage to those dealing with a declining
loved one. At ElderLife Consultants we begin by finding out what our client's
needs and desires are through a comprehensive assessment. |
| Assessments are a team effort and are completed by members of
our professional staff. They include the following areas: |

Medical
|
A review of past and present health-related problems
and treatments by a professional nurse, correspondence with client's physicians and
medication review by a Certified Geriatric Pharmacist, if needed. |
| Psychological |
A series of screenings to identify possible depression
or dementia; also screen for caregiver stress. |
| Social |
Quality of life issues are addressed; includes cultural
and spiritual components as well as hobbies and activities. |
| Environmental |
A safety assessment of current living arrangement. |
| Financial |
A review of monthly income and expenses, assets for
long-term planning, and adequate insurance coverage. (no account numbers required) |
| Legal |
A review to determine need of specific legal documents
such as Powers of Attorney, Estate Plans; review need to update existing documents
such as wills. |
| Information is gathered through personal interview, family input,
and correspondence with personal physicians. |
| The completed assessment is then used to develop options for
individuals and families based on their goals. Upon determining a client's needs
and desires we are able to determine which services or communities are appropriate.
Options and recommendations are presented and a care plan is developed by ElderLife
Consultants. |
| Clients are invited to implement the care plan themselves, or
they may contract with ElderLife Consultants for on-going
care management services. |
